Legal Requirements
- You MUST be over 18 years old to fill out these documents.
- You MUST have mental capacity to fill out these documents.
- The statutory Advance Health Care Directive MUST be witnessed by two people who are NOT your named Healthcare Agents.
- If you are in a skilled nursing facility, you MUST also get the Ombudsman to sign the form.
- The statutory Durable Power of Attorney MUST be notarized.
If these requirements are not met, the documents are invalid.
